STANDARD OPERATING PROCEDURE FOR MOTOR CAR CLAIMS:

 

In case of any motor car loss, please call MAA General Assurance, Claims Department at these numbers :

An evaluator will be sent by MAAGAP to render valuation of loss sustained.


Own Damage Claim - Documentary Requirements:

 

  1. 1. Photocopies of current Registration Certificate with Official Receipt
  2. 2. Photocopy of valid Driver’s License ID with Official Receipt
  3. 3. Police Report (original copy given by the police officer), if possible with scale drawing of the    accident,
  4. 4. Notarized Affidavit – describing how the accident happened
  5. 5. Repair estimate
  6. 6. Colored pictures showing the extent of damage and plate number.
  7. 7. Copy of Insurance policy and Official Receipt certifying payment of premium.

We will issue our Letter of Authority in favor of any accredited motor shop within twenty four (24) hours or even lesser, provided all documents are complete and in order.

For Cash Settlement claim, we require submission of proof of billing together with an official receipt and our settlement check will be released within four (4) working days.

MAAGAP reserves the right to assign an adjuster or evaluator to investigate any claim.


Theft – Documentary Requirements:

 

Accessory theft - treated the same manner as own damage claim

Vehicle Theft - should a claim arise, advise MAAGAP and report the same to the proper authorities, and secure the following documents:

  • • Alarm Sheet/Complaint Sheet
  • • Police Report
  • • Photocopy of current Registration Certificate with Official Receipt
  • • Photocopy of Driver’s Licence with O.R.

MAAGAP will assign an independent adjuster to handle valuation of loss.

Items 1 & 2 documents are being issued by the Philippine National Police Anti-Cannapping Unit (LIMBAS), Traffic Management Group based in Camp Crame, E. delos Santos Avenue, Quezon City

A sixty (60) day prescriptive waiting period is being exercised by the PNP/TMG Anti-carnapping group (LIMBAS) for possible recovery of the unit. After the lapse of the said period, client shall secure a Certification Of Non-Recovery from PNP/TMG to the effect that the unit has not been recovered as of the date of issuance, and SUBMIT THE SAME TO THE CLAIMS DEPARTMENT.


Determination of existing fair market value at the time of the loss should be made, also with the adjuster for recommendation before the lapse of the prescriptive period.

Claim check shall be released in exchange of the following documents:

  • • Original copy of Registration Certificate with current Official Receipt
  • • Deed of Sale (made by MAA Assurance) transferring ownership from insured to MAA Insurance.
  • • Release of Chattel Mortgage (if mortgaged)
  • • Duly executed and notarized Loss and Subrogation Receipt.


Third Party Property Damage Claim – Documentary Requirements:

 

In addition to the documents for an own damage claim, the following are required to support a Third Party Property Damage claim:

  • • Photocopy of Third Party’s Driver’s License with O.R.
  • • Photocopy of Third Party’s Registration Certificate with O.R.
  • • Certificate of no-claim from third party insurer.
  • • Colored pictures (provided by accredited shop)
  • • Estimate from accredited shop

Settlement of motor shop bill should be accompanied by the following:

  • • Duly signed affidavit of desistance signed by the driver of the third party vehicle
  • • Release of claim signed by the owner of the third party vehicle


Third Party Bodily Injury Claim:

 

In an event where a claim is filed under this category, an adjuster should be assigned immediately or as soon as possible.

Insured and its driver should be notified not to sign or admit any liability, immediate care and treatment for the victim is necessary. Report the incident immediately to the nearest police station.


Documentary Requirements:

 

  • • Medical Certificate (indicating injuries sustained)
  • • Medical Receipts, bills, invoices, Prescriptions and other expenses incurred related to the injuries   sustained.
  • • Such other documents necessary to prove losses sustained.

In case of Death :

  • • Certified true copy of Death or Postmortem Certificate
  • • Funeral/Burial Expense
  • • Autopsy report, if available
  • • Proof of Status as Legal Beneficiary
    • • Certified true copy of Marriage contract
    • • Certified true copy of Birth Certificate of Children (if parents are the beneficiaries)
    • • In the absence of Birth Certificates, a true copy of Baptismal Certificate or affidavits of two (2)   disinterested persons; together with the Certification from the Church or Civil Registrar’s Office   that civil records of birth are not available.

 

General Requirements:

  • • Original or copy of policy including warranties and clauses attached to it.
  • • Pictures of the damaged property.
  • • Inventory List of insured property/ies damaged with corresponding valuation of repair or replacement,   liens and encumbrances, if any, and estimated value of salvage.
  • • A brief written report as to the events that lead to the loss, including the name of witness/es if   available, location/nature of ownership of property/ies insured and the year the building was   constructed as well as the original construction cost (for building loss).
  • • Arson Investigation Report or Fire Certification
  • • PAG-ASA Report (for lightning, typhoon and other weather disturbance losses)
  • • Other documents that may be required by the adjuster.

Documentary Requirements

For Building Claim:

 

  • • Item Nos. 1 to 6 of the General Requirements.
  • • A Civil Engineer’s or Architect’s detailed estimate (obtained at the expense of the insured) to place   the building in the same state of repair as before the accident. NOTE : no contemplated   improvements may be included in the estimate, and/or proper allowance for age and depreciation,   etc. may be allowed.
  • • Appraisal report, if any
  • • Copy of the building plan, complete with dimensions and design specifications.
  • • Lease contract or agreement, if assured is not the lot owner.
  • • Certified true copy of building permit, Transfer Certificate of Title and Declaration of Real Property.
  • • Other documents that may be required by the adjuster.

For Goods and Merchandise Claim:

  • • Item Nos. 1 to 6 of the General Requirements
  • • Detailed Inventory of Articles damaged, showing the cost price of each, extent of damage and   salvage value, if any, if the risk sustained partial or water damage.
  • • Proof of damage (technical report/laboratory analysis)
  • • Inventory of the whole goods and merchandise in the building (for partial loss)
  • • Latest Inventory report, for total loss, filed with the Bureau of Internal Revenue or any Government   entity, prior to the loss.
  • • Movement reports/documents in between the last inventory report submitted and the date of loss (for   total loss)
  • • Production and movement report, for loss in the manufacturing area.
  • • Certified true copies of Income Tax Return together with the financial statements for the last three   (3)years.
  • • Books of Account, Purchases and sales invoice and all other business records (to be made   available for the adjusters review)
  • • Location of other store/s or warehouse and amount of insurance on stocks contained therein, if any.
  • • Other documents that maybe required by the adjuster.

For Machinery and Equipment / FFF Claim:

  • • Item Nos. 1 to 6 of the General Requirements.
  • • Detailed inventory of articles damaged or destroyed, showing the cost price of each, and the date   acquired
  • • Records, Invoices, Receipts if any (to be made available for the adjusters review)
  • • Building Contents lay-out
  • • Other documents that may be required by the adjuster

For Pier Impact Claim:

  • • Copy of policy with warranties and clauses attached to it.
  • • Comprehensive loss report indicating circumstances of loss.
  • • Marine Protest (to be filed with the Phil. Coast Guard within 24 hours from the date of loss).
  • • Formal Claim against the vessel owner (to be filed within 30 days from the date of loss)
  • • Underwater survey
  • • Photographs of the damaged property
  • • Complete plans and design specifications of the affected property.
  • • Detailed estimate, including bid proposals, if any.
  • • Technical Report
  • • Other documents that may be required

For Spontaneous Combustion claim:

  • • Copy of Policy with Warranties and Clauses attached to it.
  • • Insured’s report on the circumstances of the loss
  • • Laboratory analysis of burnt product for adjustment of volume.
  • • Weigh scale / ticket card indicating the volume of the burnt product.
  • • Daily Inventory / Production Records prior to the incident
  • • Shipping / Delivery Records
  • • Pictures of the burnt product
  • • Other documents that may be required.

For Robbery / Burglary Claim:

  • • Copy of insurance policy with warranties and clauses attached.
  • • Insured’s report on the circumstances of the loss.
  • • Police Investigation Report
  • • Affidavits or statements of witnesses (if any)
  • • Inventory list of insured property/ies with corresponding valuation of replacement, liens and   encumbrances, if any.
  • • Pictures showing damaged, stolen properties
  • • Other documents that may be required

For Construction / Erection All Risks:

  • • Copy of Insurance Policy with attached warranties and clauses
  • • Insured’s report on the circumstances of the loss.
  • • Drawing Plans and Design specifications
  • • Building Permit (for building losses)
  • • Detailed repair estimate or replacement proposal
  • • List of Bid Items and Quantities and any additional offers made stating dimensions and standard   prices.
  • • Inspection reports, if possible
  • • Records of Interrogation of parties concerned.
  • • Expert’s reports on soil and ground conditions, etc.
  • • Reports written by the Police.
  • • Newspaper clippings on the incident, if any
  • • Original and/or revised PERT/CPM

For IAR / Machinery Breakdown Claim:

  • • Copy of insurance policy with warranties and clauses attached
  • • Insured’s report on the circumstances of the loss.
  • • Pictures of the damaged property
  • • Technical/Engineering Report and analysis
  • • Detailed Repair estimate
  • • Quotations, Catalogs from manufacturers/suppliers.
  • • Copy of Contract Order/Agreement between insured and the contractor.
  • • Other documents that may required.

For Electronic Equipment / Business Computer Claim:

  • • Copy of insurance policy with warranties and clauses attached.
  • • Duly accomplished information for electronic equipment insurance
  • • Police Investigation Report
  • • Incident Report
  • • Pictures of the damaged equipment.
  • • Detailed repair estimates
  • • Technical report/analysis.
  • • Records, invoices, receipts if any
  • • Detailed inventory of articles damaged or destroyed showing each cost price and acquisition date.
  • • Other documents that may be required.

For Business Interruption / Loss of Profit Claim:


Non-Manufacturing

  • • Copy of insurance policy including warranties and clauses
  • • Insured’s report on the circumstances of the loss
  • • Profit and Loss statement for the fiscal year immediately preceding the loss
  • • Monthly sales report (at least two years prior to the loss)
  • • Application for business interruption insurance if any, has been filed with the insurance company.

 

Manufacturing

 

  • • Policy with warranties and clauses attached
  • • Annual financial statement for two(2) fiscal years ended prior to the loss.
    • • Income statement
    • • Balance sheet
  • • Monthly/quarterly
    • • Operating statement
    • • Inventories
    • • Production schedules
    • • Daily/hourly production in units or in sales value
    • • Gross earning value

(Comprehensive Personal Liability, Comprehensive General Liability, Products Liability, Garagekeepers Liability, Professional Liability, Manufacturer’s and Contractor’s Liability, Officer’s and Director’s Liability, etc.)

  • • Formal Notice of claim from the insured to the insurer indicating among others, the following:
    • • Late, time and place of occurrence.
    • • Description of the occurrence or circumstances of the loss.
    • • Name, personal circumstances and address of Third Party claimant/s.
    • • Names and addresses of witness/es.
  • • Copy of insurance policy with warranties and clauses attached.
  • • On bodily injury, the nature of injury sustained and the name and address of the attending physician   and/or hospital where claimant was treated and/or confined.
  • • On property damage, nature, pictures and estimate of damage
  • • If claim arises from contract, attach a copy of the contract.
  • • Attach a copy of police report, if available for all types of claims
  • • Other documents that me be required.

Cargo Claim Requirements:

  • • Copy of insurance policy with warranties and clauses attached
  • • Formal advise from insured to insurer regarding the claim
  • • The following are to be submitted :
    • •  Marine risk note and endorsement, if any.
    • •  Commercial invoice
    • •  Bill of lading
    • •  Packing lists
    • •  Copies of formal claim against the following:
    • •  Arrastre operator/contractor
    • •  Vessel’s owner/agent
    • •  Other bailee’s like custom’s broker, etc. And their respective replies.
  • • Copy of delivery receipts

  • For Claim involving shipments in Bad Order condition:
    • • Bad order certificate issued by the arrastre contractor.
    • • Copy of turn-over survey of bad order on board cargoes issued by the arrastre contractor as   attested by the vessel’s representative.
    • • Original laboratory or quality control analysis
    • • Original invoice for repair/reconditioning of bad order or damaged items.

    For claim involving short-landed shipments/spillage/leakage:
    • • Bad order certificate
    • • Turn-over survey on bad order cargoes

    For claim involving short delivered shipments:
    • • Short delivery or short landing certificate issued by the arrastre contractor certifying the   non-discharge of package/s or tally sheet of discharge.
    • • Notice of unlocated cargo filed by the insured’s custom broker against the ship owner or arrastre   contractor
    • • Permit to deliver imported goods

    For theft or Pilferage claim:
    • • Turn over survey
    • • Bad order certificate


Marine Hull Claim Documents:

  • • Copy of Marine Hull Policy
  • • Copy of vessel’s Certificate of Philippine Registry
  • • Copy of vessel’s Certificate of Ownership
  • • Copy of vessel’s Coastwise License
  • • Copy of vessel’s Departure Clearance
  • • Copy of vessel’s certificate of Inspection
  • • Master’s Note of Protest
  • • Repair estimate of damaged vessel
  • • Deck Logbooks/extracts covering the repair period
  • • Statement of the cost and expense of towing together with additional costs and expenses of putting   the vessel in two worthy condition in accordance with the Hull Underwriters’ Surveyors   recommendation
  •   Survey Reports and Bill
  • • Statement of Fees and Expenses incurred in attending the casualty and diving survey
  • • Other documents that may be required

General Requirements:

  • • Insured’s incident report on the circumstances of the loss
  • • Police report, if any
  • • Pictures of damaged property/items
  • • Repair estimate
  • • Quotation of a replacement unit
  • • Copy of policy with clauses and warranties attached
  • • Other documents that may be required
  • • Copy of insurance policy with warranties and clauses attached
  • • 201 file of the defaulting employee
  • • Certificate of employment of the defaulting employee including duties and responsibilities and period   of employment.
  • • Certificate of monies withheld due to the defaulting employee ( unclaimed salary,   commission,unpaid vacation/sick leaves and other monetary benefits )

  • • Audit report, inclusive of the following attachments:
    • • DR/PR or invoices and checks
    • • Affidavit of payment or customer’s confirmation in writing
    • • Copies of subsidiary ledgers showing non-posting of payments defalcated by the erring employee
  • • Copy of written internal control procedure
  • • Affidavit of the person who discovered the loss
  • • Copies of Insured’s memorandum, investigation report
  • • Police report or statement of witnesses, if any
  • • Legal documents for filing of estafa or civil case against erring employee such as :
    • • Demand letter
    • • Copy of affidavit/complaint and counter
    • • Status of case files and others
  • • Copy of policy schedule showing that the involved employee is covered.
  • • Copy of admission/confession letter of the defaulting employee
  • • Other documents that may be required

General Requirements:

  • • Formal notice of claim indicating among others, the following:
    • • Date, time and place of occurrence
    • • Description of the occurrence or circumstances of the loss
    • • Names, addresses of any witness
  • • Police investigation Report
  • • Statement of Witness/es (if any)
  • • Audit report/Findings with supporting documents (i.e. official receipts, provisional receipts, sales   invoice)
  • • Check vouchers, cancelled checks
  • • Payroll Sheet (for payroll robbery)
  • • Replenishment Report ( for petty cash)
  • • Collection Report (for Cash Collector)
  • • Insurance policy with attached warranties and clauses
  • • Other documents that may be required
