2.?As required by the Anti-Money Laundering Act (Republic Act 9160, and its amendments, Republic Act 9194) please present oroginals for verification and submit copies of the following documentation:

A. For individual investors:

        1. Current passport, or expired passport within the preceding two years, which          has not been cancelled

        2. Birth certificate

        3. Driver’s license

        4. Employment identification card

B. For corporations/partnerships/single proprietorships:

        1. Articles of Incorporation or Articles of Partnership for corporate accounts;         and Certificate of Business name registration issued by the Department of         Trade and Industry (DTI) for single proprietorships

        2. Latest General Information Sheet

        3. Sworn statement as to the existence or non-existence of beneficial owners

        4. Board of Directors’ or Partners’ resolution for the opening of the account         and authorized signatories, including specimen signature

        5. Valid proof of identification directors/ Partners/ Owners, and all authorized         signatories
